How do I file a claim?

Easy Online Claim Form!

Call Mountain America at 1-800-748-4302 and let them know you would like to file a claim prior to getting your phone repaired or replaced. The link to the online claims portal will be provided to you.
Go to the online claims portal within ninety (90) days of the loss or damage.
Enter all the required information on the electronic claim form.
A claims adjuster will review your submission. If no additional information is needed, you will receive an email with instructions and suggestions on approved repair/replacement facilities where you can take your device locally.
If there are questions about your submission, a claims adjuster will call you.

*Worth Ave. Group serves as the administrator of the Mobile Phone Coverage and assumes and performs administrative obligations, including but not limited to, adjudication of any claims. Worth Ave. Group's insurance policies are underwritten by the Hanover Insurance Group. There is a 30-day waiting period for claims resulting from Accidental Damage, Mechanical Failure, and Manufacture Defect. Damages that occur within this waiting period will not be covered. There is no waiting period for theft claims. The primary account holder can file a claim for any phones owned by them.
